Managing HAZOP quality is a constant challenge we face. The approach taken in most organisations is simply to stop using a facilitator after they deliver a "bad" HAZOP. This allow poor quality to perpetuate in our industry and narrows the pool of facilitators for our future use. We must also be mindful that there were also two parties in the procurement of that HAZOP and we may be in part be accountable. None of us want to buy a HAZOP at the wrong time of the project, with the wrong scope, attended by a less than ideal team and facilitated by someone who does not compliment our team.
